Each card has room for child’s photo, thumbprint, name, address, weigh, height, birth date, emergency contact numbers, doctor name and number, dentist’s name and number, insurance info, plus a medical release signature that allows for immediate medical treatment if parent is not present or can not speak for the child. Also contains self-seal laminating pouches that protect the card. No laminator needed. Replace yearly.
